Southgate residents can pick up a Registration Packet from your neighborhood elementary school.
The following is required to enroll a student.
1. Original Birth Certificate
2. Two Proofs of Residency (see back of registration form)
3. Up to Date Immunization record. (Child will not start school if missing one or more shots.)
4. District Enrollment Forms (this you get from the school office)
School of Choice parents can only pick up School of Choice applications during the open enrollment window from Central Office: 14600 Dix-Toledo Road (Southgate Municipal Building-second floor.) Central Office will notify you upon approval not the neighborhood schools.
